Person-centred practice is central to our approach, the STRIVE programme provides focus and structure, and access to a wealth of connections and opportunities.
We focus on what matters to the people we support and their families. We work in partnership with others to plan for the journey ahead, whatever that may look like. We design activities and provide bespoke, targeted support, to help achieve their identified outcomes.

Social: A spotlight on Carl
Carl enjoys the beach and is very sociable, it’s calm and peaceful surroundings have such a positive impact on his wellbeing. His team were able to find a community volunteering group that meet regularly to litter pick and clean up the beach.
With the support of his team, Carl integrated into the beach clean-up group comfortably. This has had such a positive impact for Carl, he achieves a sense of pride and accomplishment, whilst enjoying the social interaction and inclusion with people who aren’t paid to support him.
Therapeutic: A spotlight on Anthony
Anthony enjoyed getting out and about, but due to some sensory issues he would often remove his clothes, so would typically wear a onesie when out and about in public. This made his appearance different to others.
The team worked to get rid of the onesie, and with speech and language input, Anthony learned to use an electronic LAMP device to express himself. This empowered him to speak and be heard, giving him the opportunity to communicate his needs more clearly and increase periods of time wearing regular clothes rather than the onesie.
Relationship: A spotlight on Aaron
Aaron loves singing and socializing. His team supported him to join a local choir group that has extended his friendship group beyond the people he lives with.
Since joining the Choir, Aaron has taken part in events and concerts, and has been involved in extra rehearsals in preparation. This has widened his life experience and significantly increased his confidence.
Independence: A spotlight on Victor
Victor was a keen swimmer at his local pool, and his team wanted to see if these opportunities could be broadened. After speaking with the local David Lloyd gym on Victors behalf, they offered some free passes so that he could try out the facilities.
The visits were a success, and Victor joined the gym as a permanent member, and pays for the membership himself. He now goes to the gym every day and has his own set routine. His ability to cope with the busy environments has increased phenomenally and he has become more confident and independent.
Empowerment: A spotlight on Joseph
Josh has always had a love of trains and will choose to watch anything train related on his ipad. The team contacted Northern Rail who came to meet Joseph at his home to explore the possibility of him volunteering for the railway station adoption group that look after the station environment.
Joseph can be anxious about meetings, and was prepared for his person-centred planning meeting, and his chat with Northern Rail by his team, to get to the bottom of what he wanted from life and what would make him happy.
